&lt;p&gt;USB: ulpi: fix memory leak on registration failure&lt;/p&gt;
&lt;p&gt;The allocated device name is never freed on early ULPI device
registration failures.&lt;/p&gt;
&lt;p&gt;Fix this by initialising the device structure earlier and releasing the
initial reference whenever registration fails.&lt;/p&gt;</description>
